Nutrition for Life

The Long Island Association for AIDS Care (LIAAC) provides the following three food/meal components: home delivered meals, grocery/food pantry bags, and food vouchers. Clients on the home-delivered meals component will be provided with a minimum of 20 frozen meals. Each meal is inclusive of the frozen meal, juices, snacks, and frozen or fresh fruit each month. Clients on this level of service are unable to cook and shop for themselves and are deemed home restricted by their physician. Clients who receive frozen meals are initially assessed by a Registered Dietician Consultant and then reassessed approximately every 6 months to monitor nutritional progress.
Clients on the grocery/food pantry level of service receive 2 sets of pantry each month containing the ingredients for 40-50 meals. Clients on this level must attend 2 group nutrition education workshops per year, unless the Registered Dietician Consultant determines they are at nutritional risk and in need of an assessment.
The final food voucher level of service complements the above components, providing clients with a final stage of nutritional support/education before graduation. For a period of 2 months, clients are provided with monthly food vouchers sufficient to meet the AIDS Institute's food and meal content standards. The first month's vouchers will be in conjunction with a 1:1 community based nutrition education session at local grocery stores. At the second month mark, clients are given a food voucher, written guidelines and a list of allowable food items. This support, education and hands on coaching helps clients graduate from LIAAC's Nutrition for Life program within 18 months, possessing the tools needed to incorporate healthy food and nutrition into their everyday lives.
Nutrition services are provided directly to clients in their homes through predetermined delivery routes M-F 9 am-7:30 pm. Accommodations can be made for clients who are unavailable during these set times.
